AzPA Fellowship Recognition

Qualifications

To be eligible for consideration, an applicant must meet all of the following criteria:

  • Be a current pharmacist member of the AzPA.
  • Have sustained membership as a pharmacist in the AzPA for at least eight years.
  • Have a record of outstanding service to the profession through active involvement in AzPA as an elected/appointed Committee Chair OR Board Member of AzPA or a related affiliate (APF, PNA) AND
  • Have either presented at an AzPA education program OR have published an article in the Arizona Journal of Pharmacy.

FAzPA

Awarding Fellowship in the Arizona Pharmacy Association fosters and rewards demonstrated excellence in pharmacy.

Successful candidates for Fellow status will have made a sustained contribution to the AzPA and will have demonstrated a high level of excellence in pharmacy.

Details

Applications must be received by April 1st of the corresponding year.

Please complete the application in its entirety. The purpose of this application is to establish an objective basis whereby applicants can be evaluated based on the Fellow criteria.

Each application is reviewed by members of the FAzPA Selection Committee, which recommends to the Board of Directors whether an applicant should be awarded Fellow status.
