Each year the Arizona Pharmacy Association encourages its members to nominate individuals in the pharmacy profession who they feel have gone above and beyond the call of duty.

 

Look through the awards below and see how you can thank, through acknowledgement, the person who you believe deserves recognition.

Mortor and Pestel

Pharmacist of the Year

 

Selection Criteria: An Arizona pharmacist who has actively demonstrated outstanding leadership and service for AzPA or for the profession of pharmacy in Arizona during the previous year.

  • Accomplishments should include providing significant AzPA organizational leadership and should reflect ongoing personal time and effort to mentor and/or serve the needs of AzPA members and Arizona pharmacy.

 

Requirements: Nomination by an active AzPA member. Recipient must be an AzPA member and have demonstrated significant involvement in AzPA during the most recent two-year period.

Selection Criteria: This award is the highest level of recognition provided to an Arizona pharmacist who has compiled an outstanding record of community service. This service, which apart from his/her specific identification as a pharmacist, should reflect well on the pharmacy profession. Each nominee will be scored based on the following categories:

  • Professional Activities (Local, state and national pharmaceutical association memberships, offices, committees)
  • Fraternal Activities (Participation and offices held)
  • Service Clubs (Participation and offices held)
  • Church Activities (Offices, positions and participation)
  • Community Service (Elected offices, governing boards, volunteer groups, business groups, leadership positions)
  • State and National Service (Other than those related to the professions of pharmacy)
  • Military Service (Regular, reserves, national guard)

 

Requirements: Nomination by an active AzPA member. Recipient must be an AzPA member. Recipient must not have previously received this award and must not be currently serving on the AzPA Awards Committee, or as an officer of AzPA in other than an ex-officio capacity. Recipient will be asked to write a brief article that will be published in the AzPA journal.

Health System

Elias Schlossberg Award

Selection Criteria: This award is the highest level of recognition for an Arizona pharmacist who has made significant contributions to health system pharmacy in the form of sustained exemplary lifetime service that reflects positively on the profession of pharmacy.

 

Requirements: Recipient must be an AzPA member. Recipient must not have previously received this award and must not be currently serving on the AzPA Awards Committee, or as an officer of AzPA in other than an ex-officio capacity. Recipient will be asked to write a brief article that will be published in the AzPA journal.

Pharmacy Technician of the Year

 

Selection Criteria: An Arizona pharmacy technician who has excelled in the field of pharmacy technology and has demonstrated a high level of service and professionalism in pharmacy during the previous year.

 

Requirements: Nomination by an active AzPA member. Recipient must have demonstrated active involvement in AzPA during the most recent two-year period. Recipient must be a member of AzPA.

Hall of Fame

Hall of Fame Award

Sponsored by the Arizona Pharmacy Foundation

 

Selection Criteria: A pharmacist member of AzPA with at least 10 years of practice experience in Arizona who has demonstrated innovation or excellence in practice, education, or research. This person is an Arizona pharmacy pioneer who has been instrumental in advancing the profession of pharmacy by promoting the importance of innovation and integrity in the field.

  • This award may be presented posthumously to a deceased Arizona pharmacist. In the case of a deceased honoree, the individual must have been an active member of the Arizona Pharmaceutical Association (APA), the Arizona Society of Health-System Pharmacy (AzSHP) or the Arizona Pharmacy Alliance (AzPA). This person was an Arizona pharmacy pioneer who was instrumental in advancing the profession of pharmacy through advocacy, innovation and integrity in the field.

 

Requirements: Nomination by an active AzPA member. Nominating member must not be a member of the AzPA Awards Committee. Recipient must be an AzPA member or, in the case of a posthumous award, a past member of APA, AzSHP or AzPA. Recipient must not have previously received this award.

Pharmacy Youth

Pharmacy Appreciation Awards

 

Selection Criteria: An individual who has demonstrated very special or unique interest and concern for the advancement of Arizona pharmacy, as reflected by dedicating significant personal time and effort in completion of one or more special activities or services for AzPA and/or the profession of pharmacy. Examples may include activities such as teaching, committee work, organizational recruitment, legislative processes, and pharmacy-related community involvement. The recipient may or may not be a pharmacist or AzPA member.

 

Requirements: Nomination by an active AzPA member. Recipient is not eligible to win another award in the year this award is received.

Pharmacy Growth

Distinguished Young Pharmacist of the Year

Sponsored by Pharmacist Mutual

 

Selection Criteria: An Arizona pharmacist who has demonstrated significant leadership in Arizona pharmacy in the previous year. Contributions can relate to AzPA participation, activities within his/her professional practice, or community service.

 

Requirements: Nomination by an active AzPA member. Recipient must be an Arizona pharmacist, licensed to practice for 5 years or less, and be a member of AzPA.

Outstanding Pharmacist

Exemplary Patient Care Award

 

Selection Criteria: An Arizona pharmacist who provides or enables exemplary patient care, regardless of practice setting, by consistently providing exemplary patient care services, serving as a role model for contemporary pharmacy practice, or developing or fostering the development of an innovative pharmacy practice resulting in improved patient care.

 

Requirements: Nomination by an active AzPA member. Recipient must be a member of AzPA.

Representative

Outstanding Pharmaceutical Representative

 

Selection Criteria: A pharmaceutical industry representative, who serves Arizona pharmacy, routinely demonstrates a very high level of professionalism, and is committed to improving medication use by supporting their customer’s patient care goals.

 

Requirements: Nomination by an active AzPA member. Recipient must be a member of AzPA.

Excellence in Innovation

 

Selection Criteria: A nominee should be a practicing pharmacist in Arizona and should have demonstrated significant innovations in their respective pharmacy practice.

 

Requirement: Nomination by an active AzPA member. Recipient must be a member of AzPA.

Corporate

Corporate Appreciation Award

 

Selection Criteria: A corporation or business who serves Arizona pharmacy, routinely demonstrates a very high level of professionalism, and is committed to improving medication use by supporting AzPA in a significant way.

 

Requirement: Nomination by an active AzPA member.

Danger

Generations RX Champions Award

Sponsored by Cardinal Health

 

Selection Criteria: Honors a pharmacist who has demonstrated outstanding commitment to raising awareness of the dangers of prescription drug abuse among the general public and among the pharmacy community.

 

Requirement: Nomination by an active AzPA member. Recipient must be a member of AzPA.
